• Arrive 15 minutes prior to the scheduled appointment for registration

• Bring your photo ID and Insurance card(s) to your appointment

• If you arrive past your appointment time you may have to be rescheduled

• If you are unable to keep your appointment, please give us 24 hours notice to avoid a fee.  If you miss your appointment without notification, you may not be rescheduled due to appointment noncompliance policy.
